In a lab in Portland, Oregon, researchers watched something remarkable: a parent's gentle response to a baby's frown seemed to quietly rewrite that child's future.

Scientists at Oregon Health & Science University studied more than 300 mothers and their children over several years. They found that how parents notice and respond to their baby's emotional signals, like a furrowed brow or an outstretched arm, can protect children from developing attention-deficit/hyperactivity disorder, or ADHD, symptoms linked to inflammation during pregnancy.

Inflammation in a pregnant person can rise for many reasons, including infection, stress, smoking, or metabolic issues. Past research showed this inflammation raises the risk of ADHD in children. But this new study, published in the Journal of Child Psychology and Psychiatry, asked a new question: Can the way a parent cares for their baby after birth change that risk?

The answer appears to be yes.

The research team measured inflammation in blood samples during the second trimester, a critical time for fetal brain development. Then, when the children were 6 months old, they watched how parents responded to their babies' cues. Finally, they assessed the children for ADHD symptoms at age 3.

The results were striking. Children whose mothers had higher inflammation during pregnancy showed more ADHD symptoms, but only if they had received less attentive caregiving. Those who experienced what researchers call sensitive caregiving, meaning parents who noticed, understood, and responded warmly to their child's nonverbal signals, showed fewer symptoms despite the inflammation.

Lead author Hanna Gustafsson, an associate professor of psychiatry at OHSU, said the findings challenge the idea that genetics alone determine a child's mental health. "Brain development doesn't stop when a child is born; it continues throughout life and can be molded by children's experiences, their family relationships, and their community," she said.

The study suggests small moments matter more than parents might realize. Co-author Elinor Sullivan, a professor of psychiatry and behavioral neuroscience, said simple actions can make a difference: taking a few minutes to play or read together, responding quickly when a child speaks, using a warm tone of voice, or smiling at your baby.

"There is so much pressure on new parents, yet societally we provide limited support and tools," Sullivan said. "This research demonstrates that small moments of engagement and emotional connection with your child can create a significant, lifelong impact."

Researchers now plan to test interventions that might lower inflammation during pregnancy, including fatty acid supplements and mindfulness practices. The Oregon Department of Human Services is also working to connect more caregivers with parenting resources.

"Supporting caregivers throughout the transition to parenthood will not only benefit children but also create healthier families and communities for generations to come," Gustafsson said.
