When Laura was diagnosed with lupus at 22, doctors told her she might never safely have children. More than a decade later, she became one of 14 women with severe autoimmune diseases who gave birth to healthy babies after a single treatment — and without needing to resume the powerful drugs that had kept her illness in check.

The treatment, called CD19 CAR T-cell therapy, works by selectively destroying the faulty B cells that cause autoimmune diseases like lupus. Once these cells are eliminated, patients can often stop taking immunosuppressive medications entirely — medications that are normally unsafe during pregnancy.

Researchers led by Dr. Georg Schett from Germany tracked these pregnancies as part of a study involving teams from Switzerland, France, Belgium, and the United States. Their findings, published in the New England Journal of Medicine, mark the first systematic study of pregnancy outcomes after CAR T-cell therapy in autoimmune disease patients.

Of the 14 documented pregnancies, eight resulted in births. Not a single complication was reported. All babies were born healthy and developed immune systems normal for their age. Perhaps most remarkably: none of the mothers saw their underlying disease return during pregnancy, despite stopping all immunosuppressive medication.

Autoimmune diseases like lupus disproportionately affect young women during their childbearing years. Pregnancy in these patients carries serious risks — active disease can damage kidneys, the heart, and lungs, threatening both mother and child. The drugs that control these conditions are often unsafe during pregnancy, forcing women to choose between their health and their family plans.

CAR T-cell therapy may change that calculus entirely. After a single treatment, patients enter long-term remission and no longer need daily medication. This freedom from both illness and drugs supports women in making their own choices about starting a family.

The researchers describe CAR T-cell therapy as offering patients "long-term health and a self-determined life." The therapy has already shown promise in treating conditions like lupus, rheumatoid arthritis, and other autoimmune disorders that previously required lifelong medication management.

While the study is small — only 14 pregnancies — it represents a significant proof of concept. Researchers now have documented evidence that CAR T-cell therapy can give women with autoimmune diseases a path to motherhood that was previously closed to them. For these patients, the treatment may mean the difference between managing a chronic illness and truly living their lives.
