In a windowless lab at the University of Wisconsin-Madison, a group of cynomolgus macaques just did something that could one day save millions of lives: they survived dengue fever. Scientists gave the monkeys a new experimental vaccine, then exposed them to the mosquito-borne virus. Every one of them was protected. The results, published in the American Journal of Tropical Medicine and Hygiene, mark a major step toward the first vaccine that blocks all four types of dengue at once.

Here is why that matters. Dengue is not a rare disease. Spread by a bite from an infected mosquito, it sickens people across Southeast Asia, the Pacific Islands, the Caribbean, Mexico, Central and South America, and parts of Africa. Most people recover, but the virus has a dangerous twin: dengue hemorrhagic fever, a life-threatening form that strikes an estimated half a million to two million people every year. Recent outbreaks in India, Pakistan, Cuba, and even Florida show the disease keeps spreading. A working vaccine has been a long-sought goal, and no drug can cure the infection once it starts.

The vaccine itself began with InViragen, a company based in Fort Collins, Colorado. In 2006, it won a multi-million dollar grant from the National Institutes of Health (NIH), the U.S. government's main medical research agency. The four-year grant brought together scientists at the Centers for Disease Control and Prevention (CDC), the primate research center in Madison, and UW-Madison's veterinary school. Dr. Jorge Osorio, a professor at UW-Madison who also serves as InViragen's chief scientific officer, led the work. Saverio Capuano and his team at the Wisconsin National Primate Research Center supplied the macaques and handled much of the technical heavy lifting.

The results were clear. The vaccine proved safe, it trained the monkeys' immune systems to produce protective antibodies against all four dengue virus types, and none of the animals fell ill when challenged with the live virus. That is the crucial breakthrough, because previous attempts often protected against one or two types only — leaving people exposed to the others.

There is still a long road ahead. The next step is human clinical trials, and researchers will need to prove the vaccine is just as safe and effective in people. If it succeeds, InViragen says the vaccine could protect children and adults living in dengue-endemic countries — and travelers bound for those regions. For the millions of people at risk, a shot that blocks all four versions of this stubborn virus would be a quiet kind of revolution, delivered one small needle at a time.
