Behind the beeping monitors and humming machines, every hospital runs on energy, water and supplies around the clock. All that life-saving activity carries a hidden cost: in the United States, the health care sector produces about 8.5% of the country's greenhouse gas emissions — a bigger slice than many entire industries. The problem isn't just big; it's also largely invisible, because most hospitals have no standard way of measuring what they use and throw away. Now a team of researchers wants to change that with a simple idea: track the environment the same way hospitals already track patient safety.

Writing in The Lancet: Planetary Health, researchers led by UC Davis Health proposed a shared framework of sustainability measures that hospitals could adopt nationwide. "Hospitals need to track their environmental impact," said corresponding author Patrick Romano, a professor of internal medicine and pediatrics at UC Davis Health. His team reviewed the environmental measures health care organizations already use, grouping them into five categories: greenhouse gas emissions, water use, energy use, travel and solid waste. Every one of those measures matters, the researchers found, but some are hard to track because reliable data just doesn't exist — counting patient and staff travel or emissions buried deep in supply chains can be especially tricky.

To keep things practical, the authors recommended a minimum set of three metrics every hospital can report: greenhouse gas emissions from direct operations and purchased energy, water consumption and solid waste production. Crucially, they suggest adjusting these numbers for how many patients a hospital treats, so a small rural clinic and a huge city medical center can be compared fairly. That fairness matters, because one reason sustainability efforts have stalled is that hospitals can't easily see how they stack up against one another. Fragmented, one-off pledges are hard to measure and even harder to improve.

The deeper argument of the paper is that caring for patients and caring for the planet belong together. "Measuring and reporting patient safety has helped drive major improvements in care over the past two decades," Romano said. He argues the same approach could now push hospitals to slash waste and pollution while improving community health. Rather than building a whole new reporting system, first author Michael Padget, an epidemiologist at Massachusetts General Hospital, suggested folding these measures into the quality checks hospitals already undergo, so environmental performance sits right alongside safety, effectiveness and equity. Co-authors include Gregg Furie of Brigham and Women's Hospital, Dionne Kringos of the University of Amsterdam, and Jonathan Slutzman of Massachusetts General Hospital and Harvard Medical School.

The vision is quietly ambitious: a future where a hospital's water bill and waste bins are watched as closely as its infection rates, and where the same habit of honest measurement that made care safer now makes it greener too.
