When Dr. Aoibhín Sheedy set out to design a better way to treat ovarian cancer, she started with a simple but frustrating reality: the doctors knew that delivering drugs directly to tumors worked better, but the tools available simply weren't built for the job. Now, she and her team at the University of Galway in Ireland may have changed that. They have created a small, flexible implant that sits inside the abdomen and can deliver cancer treatments right where they are needed, again and again, without requiring additional surgery. The device even lets doctors peek inside the body in real time to see how the tumor is responding to treatment. The research, published in the journal Device, was a collaboration between scientists at the University of Galway, the University of Minnesota, MIT, and the Wyss Institute. Dr. Sheedy, who led the project as a PhD graduate with CÚRAM (the Research Ireland Centre for Medical Devices), said the goal was to build something designed specifically for ovarian cancer patients. The implant is made from a soft, porous material that molds naturally to the body's internal contours. A thin tube connects it to a small port on the outside of the skin, allowing doctors to refill it with fresh therapeutic agents whenever needed, without cutting the patient open again. In laboratory tests, the device kept working for up to 70 days with no complications and showed much better tumor control over time compared to standard treatments. One of the most exciting features is the ability to monitor the disease. By applying gentle suction through the same port, doctors can draw fluid from inside the abdominal cavity and see what's happening with the tumor without any extra procedure. "What excites us most is the two-way nature of this approach," said Dr. Eimear Dolan, an associate professor of biomedical engineering at the University of Galway who co-led the research. "It lets us create a picture of what's happening inside. Clinicians could use this to track how the immune cells are performing, whether the tumor is responding, and then adapt treatment accordingly." Ovarian cancer is often caught late because symptoms like bloating and stomach pain are easy to dismiss. Current treatments usually involve surgery followed by drugs, but targeted options remain limited. The Galway team designed their implant to handle living cell therapies, which are among the most powerful but hardest to deliver treatments available. Dr. Martin Felices, co-senior author and associate professor of medicine at the University of Minnesota, called the system a significant step forward. "The delivery system allows for safer repeated delivery of cellular and biologic therapies in the context of the peritoneal cavity," he said. "It also allows us to sample the effects of those therapies in the tumor microenvironment in real time, which is extremely exciting." The researchers emphasize that the work is still in early stages, but the two-way delivery system represents a new kind of tool: one that treats and listens at the same time. For patients facing a disease that is often diagnosed too late, that combination of smarter treatment and real-time feedback could eventually change the story.
