The fungus Cryptococcus quietly invades the bodies of hundreds of thousands of people with weakened immune systems every year. In most of them, it causes no obvious symptoms at first. But in others, it travels to the brain and spinal cord, causing a deadly form of meningitis. The disease kills about 110,000 people globally each year and makes up nearly one-fifth of all HIV-related deaths. For those who develop meningitis, even with treatment, between one-third and one-half will die within 10 weeks — a death rate comparable to some of the most feared outbreaks in history.
But researchers may have found a way to stop the disease before it reaches the brain. A clinical trial called EFFECT tested whether a two-drug combination could save more lives than the standard single-drug treatment when given earlier, before meningitis sets in.
Scientists recruited 604 people living with HIV across 11 sites in South Africa and Tanzania. All had traces of the Cryptococcus fungus in their blood — a warning sign that the disease could spread — but showed no symptoms of meningitis yet. The researchers split participants into two groups: one received fluconazole alone, the standard treatment, while the other got fluconazole combined with a second drug called flucytosine. Everyone took their pills for two weeks, then moved to a lower dose for eight more weeks.
The results were striking. After six months, people taking the two-drug combination were 21% less likely to have died from any cause. In the combination group, 40 out of 304 people died — a 13% death rate. In the group receiving only the standard drug, 50 out of 300 people died, a 17% death rate. Among those who took all their medication as prescribed, the benefit was even clearer: the combination therapy cut the risk of death by 29%. And in a smaller group of 134 patients whose blood showed high levels of the fungus, the two-drug approach reduced the risk of death by nearly half.
The researchers caution that the study was not large enough to prove definitively that the treatment itself caused the improvement. A bigger trial would be needed to confirm the finding with certainty. However, no significant difference in serious side effects was found between the two groups, suggesting the combination is safe.
Dr. Síle Molloy, a co-lead researcher from the School of Health & Medical Sciences, said the findings represent a turning point. "This work marks a major step forward in our understanding of how to prevent avoidable deaths from cryptococcal meningitis in screening programs worldwide," she said. "The two-drug therapy was safe and was associated with lower death rates, providing encouraging evidence to support further research and implementation studies in real-world settings."
The implication is significant: if clinics can catch cryptococcal disease early through simple blood tests, they might be able to stop it from ever reaching the brain — and save tens of thousands of lives each year in the process.
