When researchers finished studying the gut bacteria of 778 women in South Africa and Kenya, they faced a choice: pack up and leave, or hand back the results. They chose the second option—and in doing so, they may have changed how science gets done across the continent.
The women received personalized reports about their own gut microbiomes—the trillions of tiny bacteria living inside each of us—through a project linked to the AWI-Gen 2 study, the largest survey of African gut bacteria ever conducted. Published in the journal mSystems, the study showed that returning such complex health information to participants is not only possible but genuinely welcome.
"Our findings demonstrate that even highly complex scientific information can be shared effectively when researchers work closely with communities and adapt communication to local cultural contexts," said Dr. Luicer Olubayo, a co-first author from the Sydney Brenner Institute for Molecular Bioscience (SBIMB) at Wits University in Johannesburg.
The original AWI-Gen 2 study, published in Nature in 2025, had already made waves by discovering 1,005 bacterial genomes—entire sets of genetic material—that were completely missing from existing scientific collections. Researchers found that where people live, how they live, and what they eat all shape the unique mix of microbes in their guts. These findings challenged assumptions, since most past microbiome research focused on people from Europe and North America.
Returning results to participants is uncommon in studies like this, especially in lower-resource settings. The science is technical, the costs add up, and explaining it in ways that make sense across languages and cultures is genuinely hard. Critics worried that giving people personal data might cause confusion or worry.
So the team got creative. They built culturally appropriate teaching materials, held face-to-face feedback sessions, and spoke with women in their own languages. Participants learned about their gut diversity, how they compared to others in their community, and how diet and lifestyle connect to gut health.
The response was overwhelmingly positive. Women said the information helped them understand their own bodies better and made them more trusting of the research process. Some reported it encouraged them to eat more fiber-rich foods like vegetables and whole grains.
"The study shows that participants value receiving personalized research results and that returning those results can strengthen relationships between researchers and communities while promoting a more inclusive and equitable research process," said co-first author Dr. Claudine Nkera-Gutabara of the SAMRC/Wits Aging African Adult Research Unit.
Dr. Dylan Maghini, who launched the return-of-results project while working between Wits and Stanford University, said the goal was simple: show that excellent science should also give back to the people who make it possible. A companion paper in Nature Microbiology echoed this, arguing that sharing results with participants should become a standard part of microbiome research worldwide—not an afterthought.
For the women who took part, the experience may have been the first time science gave them something tangible in return.
